Well, I'm a bit of an idiot.

The config was all well. It was the config in mana+ (i.e. the client) that was wrong. I'd selected the wrong sort of server. Doh. But at least I now know the effect of doing that .

With manaserv, there is the option of using MySQL or SQLite. Does tmw-eathena use a backend SQL server of any description?
