Regnum Online, it is free to play, it have also premium items that are not really necessary, you pay for them if you just want rides or fancy things.

No, it isn't. But currently it is very easy to pirate as long as you don't want to play online.

No, it isn't. You can play a stone-age version for free on minecraft.net, but you have to pay for the newer version (Beta 1.2_02)... theoretically.
