why not? diseases are an interesting feature. i played another mmorpg which had a disease feature, too. but the difference was that it was planned by the game designers and that you could make a quest to make yourself immune to it.
could be funny to have an infectious disease, too. but the players should of course be able to heal themself from it. would be interesting to watch how fast an epidemy gets around the server and if the players can manage to get rid of it.

It's interesting but killing all low level Players is not a good idea...
Such a thing shouldn't really kill. It should be very annoying though.
But in my opinion it shouldn't affect NPCs
yes, or at least the npcs should die too. so when at some point everyone's dead at the same time, the epidemy vanishes. they really outdid themselves there, with infected -immortal- npcs all over the cities.
