In-game moderation?

Many, if not most, times I log in to play, it's only a matter of time until I run across someone breaking the rules or otherwise being a jerk to the detriment of others' enjoyment of the game. Today someone with no apparent other goal was walking around shouting swearwords in all caps.

There needs to be some sort of in-game authority who can deal with people who don't respect the rules and other players. A forum-based system is simply too slow.

No game I've ever played just lets the assholes take over.

I suppose I'm rather making a complaint about the abuse report system than making a specific abuse report.
