It's based on the Classic theme, as in the colourscheme, but has a torn, lumpy look to everything. This is difficult to pull off on things that change size, but if I do it right it should look nice.
So, first theme: no name yet, I've just got it called 'enchilado' but it's easy to rename it whatever you like. A lot of the stuff is still the same as Classic - that's because I've only done a small amount of work on it, eventually it will have a different image for everything. If you try it, please give me tips on anything to do with it.
Using the theme with 4144's patch
If you don't have his patch, you can still use it by placing it under /tmw/data/graphics/gui/. However this has extra .xml files, and no window.xml, so I don't know exactly what you'd have to do there.
Under Linux (or Ubuntu, anyway) goto the 'mana' folder: /usr/share/games/mana/, probably. Go /data/themes/ from here, and there should be a few folders. Extract enchilado.zip here, and the next time you start your client "enchilado" should be listed under Setup -> Theme.
I imagine it would be very similar under Windows, only the /mana/ folder would be in a different place.
